Arranging a test

Arranging a test is not a difficult process, but it will pay to read these steps and follow them carefully.

1. Obtain application forms - For paternity testing, relationship testing, prenatal or immigration testing, begin by downloading the appropriate form.

2. Complete application forms - Complete the application form, nominating the doctor you wish to collect your samples. If you prefer, we can refer you to a pathology centre. Also nominate the medical or legal professional who will receive your results. If you have any problems completeing the form, call us and we will help.
3. Payment - Fax your completed application form with credit card payment to DNA Labs on (02) 9221 9272 or mail it to:

4. Forms - Await a letter from us stating that the collection materials have been sent to your nominated doctor or pathology centre. You will also receive a Form 2 Affidavit which must be completed on the day of sample collection, and witnessed by a Justice of the Peace or Solicitor.
   Form 2 - Child (to be completed by the parent/guardian of a child under 18) (PDF 75K)
   Form 2 - Adult (to be completed by all adults being tested. (PDF 75K)

   Do not fill in these forms until the day of the collection

5. Photos - Have two passport-size photos taken of all persons who will be involved in testing.
6. Collection day - Fill in the Family Law Act forms. Attach one photo and have them witnessed by a Justice of the Peace or a solicitor. Give the other photos to the doctor so they can attach them to their form. Have the samples collected. The doctor or pathology centre will forward your samples and completed forms to us via courier.
7. Results - Results of your tests will be forwarded to the medical or legal professional you have nominated.
