Forms
All forms relating to your DNA test are a legal requirement.
Form 2 is an Affidavit and must be sworn in front of a Justice of the Peace on the same day as your sample is collected. A passport-sized photograph of each person participating in the testing will also need to be provided to the JP, to be verified and attached to the Form 2. Please arrange to have it completed and witnessed prior to attending your sample collection appointment. Form 2 is sent to you at a later date when we write to you to confirm your sample collection details. The mother (or legal guardian) must complete Form 2 on behalf of a child under 18 years.
DNA Labs distributes the appropriate forms with the collection materials to your doctor or pathology centre. The sample collector will complete Form 4, and attach photographs to it for proof of identity (these photographs are in addition to those attached to the Form 2).
Care must be taken to complete these forms fully. Incomplete or incorrect forms will require re-collection of samples and this will result in additional collection costs for you.
